WORKING PAPERS
1. Employer must complete an Intention to Employ Minor form; form must be signed by
parent/guardian.
2. Student must bring completed form to S.K. School Administration Building at 307
Curtis Corner Road with acceptable proof of age.
3. The School Department will issue either a Special Limited Permit to Work (ages 14-
15) or Certificate of Age Form (ages 16-17).
